HYPNOTHERAPY CONTRACT AND TERMS OF AGREEMENT.
Confidentiality and GDPR
At [Angles Therapy] all data and information are stored securely and in compliance with EU Data
Protection Laws. All information shared and discussed within your [the client] sessions is confidential
and will not be shared unless requested and approved by you [the client]. Other occurrences of
sharing information/documents regarding you [the client] are listed below;
Disclosure of unreported abuse on a young person (below age of 18), or a vulnerable adult.
Risk of significant harm to self or others.
Suspected involvement in an act of terrorism, trafficking or money laundering.
Liaison with Health Professionals as part of a coordinated health care package.
Court order.
Where possible and if appropriate any breach of confidentiality will be discussed with you before action is taken. Documents will be stored in a locked cabinet that only we [Angles Therapy] can access. After our sessions have come to an end, these documents and any subsequent notes made regarding you [the client] will be stored for up to 12 months and then destroyed, unless specified otherwise by you [the client].
Location
Your [the client] sessions will be held in our [Angles Therapy] therapy room at 36 Bellevue Road, ME12 2JF. This room is locked when not in use and only accessible by us [Angles Therapy].
Fees and Payment
The timeline of payment will be no less than 24 hours before the booked session.
Failure to attend without notification: Full Fee to be paid.
• Cancellation with more than 24 hours’ notice: No Fee.
• 50% non-refundable deposit for certain services, i.e Smoking/Vaping Cessation, Weight
Management, Hypnotic Gastric Band or block bookings to be taken on the day of booking the appointment.
Structure and Missed/Cancelled Appointments
Sessions will last 60 minutes and will begin and end at the agreed times. There will be time at the beginning of each session to reflect on how you [the client] felt/have been feeling since the last session, and what you [the client] have experienced or if you [the client] have any questions. There will also be time at the end of the session to bring you [the client] back to full awareness
comfortably and safely, and to give you [the client] another opportunity for any questions or to express any concerns. You [the client] are welcome to bring water with you to the sessions, but we [Angles Therapy] ask you to refrain from bringing any hot drinks, alcoholic drinks, or any fizzy drinks or food. You [the client] will not be able to smoke/vape/eat during sessions.
• If you [the client] need to cancel or reschedule an appointment, this will need to be done as
far in advance as possible, no later than 24hrs in advance.
• Cancellations made within 24 hours will be charged.
• You [the client] will not arrive under the influence of drugs or alcohol.
• It is your [the client] responsibility to inform us [Angles Therapy] of any changes to contact details/address.
• If you [the client] miss 3 sessions in a row without prior notice, you will be discharged. If you
[the client] wish to re-engage with us [Angles Therapy] then you [the client] may be placed on a waiting list.
Contact between sessions
Contact between sessions should be (1) for you [the client] to inform us [Angles Therapy] of any
cancellation or postponement of appointment requests., or (2) for us [Angles Therapy] to inform you
[the client] of any cancellation or postponement of appointment changes., (3) for you [the client] to
inform us [Angles Therapy] of delays/lateness to your [the client] appointment., (4) for us [Angles
Therapy] to send reminders of your appointment and if payment is due.
*Please note if you cannot reach us [Angles Therapy] by telephone (or chosen form of
communication) it may be because I am with a client or not working. Please leave a message, text, e-
mail, and we will endeavour to get back to you [the client] within 24hrs.*
Code of Ethics
At [Angles Therapy] it is imperative that all client’s documents, information, and details are stored securely and kept confidential. Clear boundaries are put in place, so both you [the client] and us [Angles Therapy] do not cross the line between client/therapist. We [Angles Therapy] will continue to undergo CPD, to ensure the best and most accurate forms of therapy are given to you [the client].
We [Angles Therapy] endeavour to be trustworthy, empathetic, sincere, and respectful. Discussions with Supervisors regarding clients will be kept confidential i.e., only client ‘codes’ will be used, there will be no use of names or any other identifying information.
We [Angles Therapy] will not carry out sessions for or work with clients that present any of the
following;
• Suffering from epilepsy or seizures.
• Asthma or any other breathing difficulties.
• Heart disease.
• Have a clinical diagnosis (or undiagnosed) psychosis or BPD.
• Have diagnosed or undiagnosed depression (past or present and medicated or not).
• History or current experience with abuse/trauma.
